If your teenager is in need of orthodontic treatment, you might automatically think braces. But while this decades-old appliance is quite effective, it isn’t the only “tooth movement” game in town any more. Clear aligners are another choice your teenager might find more appealing.
Clear aligners are a sequential set of computer-generated plastic trays that are worn by a patient one after the other, usually for about two weeks per tray. The trays are fabricated using 3-D computer modeling of the patient’s mouth, each one slightly different from the last to gradually move teeth to the desired new positions.
So, why choose clear aligners over braces?
They’re nearly invisible. Because they’re made of a clear polymer material, they’re not nearly as noticeable as metal braces. In fact, they may go completely unnoticed to the casual observer.
They’re removable. Unlike metal braces, which are fixed in place by an orthodontist, clear aligners can be removed by the wearer. This makes brushing and flossing much easier, and they can also be removed for eating or special occasions. That said, though, they should be worn at least 20 to 22 hours each day to be effective.
They’re becoming more versatile. With some complicated malocclusions (poor bites), braces and other orthodontic appliances may still be necessary. But innovations like added power ridges in clear aligners can more precisely control which teeth move and which don’t. This has greatly increased the number of poor bite scenarios where we can appropriately use clear aligners.
If you’d like to consider clear aligners, just remember they require a bit more self-discipline on the part of the wearer than braces. And once the treatment finishes, they’ll still need to wear a retainer just as with metal braces to help keep the repositioned teeth from reverting to their old positions.

Physical pain is never pleasant or welcomed. Nevertheless, it’s necessary for your well-being—pain is your body telling you something isn’t right and needs your attention.
That fully applies to tooth pain. Not all tooth pain is the same—the intensity, location and duration could all be telling you one of a number of things that could be wrong. In a way, pain has its own “language” that can give us vital clues as to what’s truly causing it.
Here are 3 types of tooth pain and what they might be telling you about an underlying dental problem.
Sensitivity to hot or cold. If you’ve ever had a sharp, momentary pain after consuming something hot like coffee or cold like ice cream, this could indicate several causative possibilities. You might have a small area of tooth decay or a loose filling. You might also have an exposed root due to gum recession, which is much more sensitive to temperature or pressure changes. The latter is also a sign of periodontal (gum) disease.
Acute or constant pain. If you’re feeling a severe and continuing pain from one particular area of your teeth (even if you can’t tell exactly which one), this could mean the pulp, the tooth’s innermost layer, has become infected with decay. The pain is emanating from nerves within the pulp coming under attack from the decay. To save the tooth, you may need a root canal treatment to remove the decayed tissue and seal the tooth from further infection. You should see your dentist as soon as possible, even if the pain suddenly stops—that only means the nerves have died, but the decay is still there and threatening your tooth.
Severe gum pain. If there’s an extremely painful spot on your gums especially sensitive to touch, then you may have an abscess. This is a localized area of infection that develops in the gums either as the result of periodontal (gum) disease, or an infection spreading from the tooth pulp into the gum tissues. You’ll need to see a dentist immediately for both pain relief and appropriate treatment (including a possible root canal) to heal the abscessed tissue.

Periodontal (gum) disease is a bacterial infection, which if left untreated could cause gum recession, bone loss and eventually tooth loss. Caused mainly by plaque left on tooth surfaces from poor hygiene practices, the deeper the infection spreads below the gum line, the more difficult it is to treat.
One possible scenario involves parts of a tooth’s root structure known as furcations. These are branching forks formed during the early development of teeth with multiple roots where they take different paths from the base of the crown. As gum disease spreads around the root it may cause different degrees of bone loss at the point of the branch.
It’s imperative when treating gum disease to uncover and remove any bacterial plaque or calculus (hardened plaque deposits) found, including below the gum line. To address bacterial plaque at the root level, it’s important to first determine if bone loss has involved the furcations (where the roots separate, also referred to as a “furcation invasion”) and to what degree.
We usually classify this degree of involvement in three classes: Class I, the invasion has created a groove in the furcation, but minimal significant bone loss; Class II, the bone loss has extended into the furcation by at least two millimeters; or Class III, the bone loss extends completely from one end of the furcation to the other (or “through and through”).
Depending on the class, cleaning plaque and calculus from furcations and then maintaining them thereafter can be quite challenging. We may need to use specially shaped hand instruments known as scalers or curettes to reach and clean root surfaces, or ultrasonic scalers that use high-frequency vibrations and streaming water to loosen and flush away plaque debris. It may also prove helpful, though limited, to apply antimicrobials or antibiotics to the area to help limit the levels of bacteria.
Disease damage around furcations may also require surgical treatment to encourage new tissue and bone growth in the area. Surgery can also help make the area more accessible to future cleaning and maintenance, both for you and us. Renewed hygiene practices on your part and regular cleaning and checkups with us will help ensure that the situation involving your tooth roots can be kept under control and your tooth preserved for many years to come.
